Care management for dementia person in the home. (This equals about 8 hours of care consulting a month.)

Care management for a person in a facility. (This equals about 4 hours of care consulting a month.)

Senior Advisor
Monthly availability for either one or two hours to problem-solve, guide, and manage care issues and caregiver support.
