Evaluation

I offer a range of dementia care management by comprehensively evaluating current status with detailed written recommendations for future health outcomes. (Approximately 4 hours)

In-Home Care Management

Care management for dementia person in the home.  (This equals about 8 hours of care consulting a month.)
$400/month 

Facility Care Management

Care management for a person in a facility.   (This equals about 4 hours of care consulting a month.)
$250/month  

Senior Advisor

Monthly availability for either one or two hours to problem-solve, guide, and manage care issues and caregiver support.
$50/month tier 1
$100/month tier 2   

Additional Care Management Hours

Attendance at Physician Appointments

"An ounce of prevention is worth a pound of cure" is never truer than dealing with dementia because dementia can be prevented! That's right; dementia can be prevented, starting in the home.
